`residing at Lindsay Park Housing, and an issue in this case is defendant's prior notice of the
`presence of lead-based paint. Ms. Thomas was the public health sanitation who inspected the
`infant plaintiffs apartment and, as such, is a potential witness in this- action. The subpoenaed
`records are therefore relevant to such issues.

`TAKE FURTHER NOTICE, that failure to comply with this ~ubpoena is punishable
`as a contempt of Court and shall make you liable to the person(s) ppon whose behalf this
`subpoena was issued for a penalty not to exceed fifty dollars and aU damages sustained by
